The BHI COCM Typically Is Supported By HealMed Within The Following Key Elements
Primary care providers screen patients for behavioral health needs and conduct initial assessments to identify individuals who may benefit from additional mental health support through HealMed clinician dashboard and patient mobile application.
Primary care providers work closely with behavioral health specialists, such as psychiatrists, psychologists, or licensed clinical social workers, to develop and implement care plans within HealMed clinician dashboard. This can involve regular communication, consultation and if necessary even remotely via video based tele consultation, and shared decision-making.
A designated HealMed care coordinator (often a nurse or a case manager assigned by HealMed), helps facilitate communication and coordination between the primary care team,HealMed behavioral health specialists, and other community resources.
